Download the appropriate
form you would like to
register your child for.		 Fill out the form completely.
If you are submitting a form
for a new child please attach
a small photo and a copy of their birth certificate.		 Please submit all
forms to your team manager.

Timely registration allows the club ability to form teams, build rosters, obtain coaching and
produce schedules which involve other leagues in an efficient manner. Your cooperation is greatly
appreciated! Fees do not include uniform costs. 

Teams will remain intact whenever possible, provided registrations are received on time. New players are assigned to teams on a first-come, first-served basis.

NOTE: A small 1X1 photo must be attached to the registration. New players must include a copy of
their birth certificate.
