Please fill out the above form for Developmental teams (U5 - U18) Cross Quad (U9) or Option 1 (U10-18). This form serves for all Developmental teams. If you are unsure what level of play is best suited for your child, please contact us.

Timely registration allows the club ability to form teams, build rosters, obtain coaching and
produce schedules which involve other leagues in an efficient manner. Your cooperation is greatly
appreciated! Fees do not include uniform costs.

Registration is for one season only, players must register each season. Teams will remain intact
whenever possible, provided registrations are received on time. New players are assigned to
teams on a first-come, first-served basis.

NOTE: A small 1X1 photo must be attached to the registration. New players must include a copy of
their birth certificate.
